Abstract
Cilj istraživanja: Određivanje učestalosti najčešće izoliranih bakterija iz uzoraka urina, brisa uretre i ejakulata u izvanbolničkoj muškoj populaciji liječenih u razdoblju od 1.1.2014. do 30.6.2014. u OBŽ, odrediti osjetljivost bakterijskih izolata na antibiotike te prikazati vrstu uzoraka za mikrobiološku obradu, raspodjelu dobi ispitanika s izoliranim uzročnicima. Nacrt studije: Studija je ustrojena kao retrospektivno istraživanje. Materijal i metode: U istraživanje je uključeno 688 izolata urina, brisa uretre i ejakulata obrađenih u mikrobiološkom laboratoriju Službe za mikrobiologiju Zavoda za javno zdravstvo Osječko-baranjske županije, u razdoblju od 1.1.2014. do 30.6.2014. Provedeno je u 2 mjeseca. Tijekom promatranog razdoblja uzorci urina ispitanika mikrobiološki su obrađeni prema uobičajenom protokolu u skladu s pravilima struke, standardiziranim metodama izolacije i identifikacije bakterija. Ispitivanje osjetljivosti na antibiotike izvršeno je prema preporuci i uputama EUCAST-a. Rezultati: E. coli je najčešće izolirana bakterija, sa prisutnošću u 47,2% izolata, zatim E. faecalis sa 35%, te U. urealyticum sa 17,9%. U dobnoj skupini ispitanika od 46 do 65 godina bilo je zabilježeno 25,4% izolata. U. urealyticum češće je izolirana u mlađoj populaciji, dok su E. coli i E. faecalis češće izolirani u starijoj populaciji. Najviše izolata bilo je iz uzoraka urina (74,3%), a najmanje briseva uretre (18,9%) u kojima je najčešće bila izolirana U. urealyticum. U E. coli je zabilježena značajna rezistencija samo na ampicilin, E. faecalis bio je dobro osjetljiv na najčešće korištene antibiotike u liječenju i prevenciji, a U. urealyticum je bila dobro osjetljiva na doksicilin, ali i na azitromicin (94,3% izolata), samo je na eritromicin zabilježeno više izolata sa smanjenom osjetljivošću. Zaključak: Naši podaci pokazuju, što odgovara preporukama u Smjernicama ISKRA-e, da ciprofloksacin može biti prvi izbor u ambulantnom liječenju akutnih IMS, a da se kao alternativa mogu koristiti amoksicilin s klavulanskom kiselinom, cefuroksim, ceftibuten, te cefiksim.

Objectives: Determining the frequency of the most frequently isolated bacteria from samples of urine, urethral swabs and semen in the outpatient male population treated in the period from 1.1.2014. to 06.30.2014., to show the distribution of bacteria, depending on the age and type of sample, and to determine the sensitivity of bacteria to antibiotics. Study design: The study is structured as a retrospective study. Materials and methods: The study included 688 isolates of urine, urethral swabs and semen processed in the laboratory of Department of Microbiology of the Institute of Public Health of Osijek-Baranja County, in the period from 1.1.2014. to 30.6.2014. The study was performed in 2 months. During that period urine samples were microbiologically processed according to the usual protocol in accordance with professional standards, standardized methods of isolation and identification of bacteria. Susceptibility testing to antibiotics is carried out by EUCAST recommendation. Results: E. coli is the most commonly isolated bacteria, present in 47,2% of all isolates, then comes E. faecalis with 35%, and U. urealyticum with 17,9% presence. 25.4% isolates were positive in the age group 46-65 years. U. urealyticum more frequently affects young adults, while the other two bacteria older. Most of positive isolates were urine samples (74.3%) and the least urethral swabs (18.9%) which are the most common sample positive for U. urealyticum. E. coli is the most resistant to ampicilin and U. urealyticum to erythromycin. E. faecalis was sensitive to most frequently used antibiotics in therapy and prevention. U. urealyticum was high-sensitive to doxycycline and azithromycin (94,3% isolates were sensitive). Conclusion: Our data indicate, same as Guidelines ISKRA, that ciprofloxacin is the first choice in the outpatient treatment of acute UTI, and as an alternative can be used amoxicillin clavulanic acid, cefuroxime, ceftriaxone, and cefixime.
