Abstract
Ciljevi istraživanja: Utvrditi ukupni broj pacijenata odrasle dobi koji su razvili infekciju, utvrditi postoji li povezanost pojave infekcije s dokazanim i potencijalnim čimbenicima rizika za infekciju rane na nozi poslije operacije revaskularizacije miokarda. Nacrt studije: Retrospektivno istraživanje. Bolesnici i metode: U istraživanju su sudjelovali svi bolesnici odrasle dobi kojima je rađena operacija revaskularizacije miokarda na Odjelu kardijalne kirurgije Kliničkog bolničkog centra Osijek u 2012. g. Koristila se medicinska dokumentacija pohranjena u arhivu Odjela za kardiokirurgiju. Težina infekcije se određivala po ASEPSIS sustavu, a parametri koji su se pratili su dob, spol, ITM, pušenje, dijabetes, EuroScore, periferna vaskularna bolest, reoperacija, vrijeme ekstubacije, duljina klemanja aorte i trajanja EKC, razina prepariranja VSM, uporaba IABP te duljina hospitalizacije. Rezultati: Od promatranih 170 pacijenata koji su podvrgnuti kirurškoj revaskularizaciji miokarda, 27 je razvilo infekciju. U ovisnosti o postojanju infekcije noge postojala je statistički značajna razlika u EuroScore-u (p = 0.032), ekstubaciji (p = 0,035), prijašnjim operacijama (p = 0,023) i duljini hospitalizacije (p = 0,009). Nije postojala statistički značajna razlika u ostalim parametrima, od kojih su značajni spol, ITM, postojanje PVB, korištenje IABP, korištenje LIMA-e, vrijeme EKC, vrijeme klemanja i prijeoperacijska EF. Zaključak: Od potencijalnih čimbenika ustanovili smo postojanje povezanosti između povišenog EuroScore-a, reoperacije, vremena ekstubacije i duljine hospitalizacije za povećani rizik od infekcije noge na mjestu uzimanja v. saphene magne. Nije dokazana korelacija poznatih i ostalih potencijalnih rizičnih čimbenika na učestalost infekcije.

Objectives: To determine total number of adult patients in the study with leg infection, to determine if there is a correlation between proven and potential risk factors and incidence of leg infection after CABG surgery. Study design: A retrospective study. Patients and methods: The study involved all adult patients with artery bypass graft surgery in 2012. Medical records stored in the archives of the Department of cardiac surgery in Osijek were used. All leg wounds were assessed using the ASEPSIS scoring system, and parameters that were monitored included age, sex, BMI, smoking, diabetes, EuroScore, peripheral vascular disease, reoperation, time of extubation, duration of aortal clamping and duration of extra-corporeal circulation, level of saphenous vein harvesting site, use of the intra-aortal balloon pump and length of hospitalization. Results: From the observed 170 patients undergoing surgical myocardial revascularization, 27 patients developed a leg infection. Depending on the existence of leg infection there was a statistically significant difference in EuroScore (p = 0.032), time of extubation (p = 0.035), previous surgery (p = 0.023) and length of hospitalization (p = 0.009). There was no statistically significant difference in other parameters, such as gender, ITM, presence of peripheral vascular disease, use of the intra-aortal balloon pump, use of left internal mammary artery, extra-corporeal circulation time, duration of aortal clamping or preoperative ejection fraction. Conclusion: Four potential factors, including high EuroScore, reoperation, extubation time and length of hospital stay have proved the increased risk of leg infection in saphenous vein harvesting site. There is no proven correlation between previously known risk factors and the incidence of infection.
