Abstract
Ciljevi istraživanja: Utvrđivanje ukupnog broja pacijenata, spola i dobi ispitanika, zastupljenosti i uspješnosti pojedinih kirurških metoda, čimbenika rizika za nekompletnost kirurške revakularizacije miokarda, utvrđivanje razlika u uspješnosti revaskularizacije ovisno o vrsti operativnog zahvata, broju ugrađenih i planiranih premosnica. Nacrt studije: Istraživanje je retrospektivno. Ispitanici i metode: Pregled povijesti bolesti, operacijskih lista i otpusnih pisama ispitanika podvrgnutih kirurškoj revaskularizaciji miokarda na Odjelu za kardijalnu kirurgiju u Kliničkom bolničkom centru Osijek od 1. siječnja 2010. do 31. prosinca 2014. Rezultati: Od ukupno 798 ispitanika, 597 bilo je muškog spola, a 201 ženskog spola. Od ukupno 798 kirurških revaskularizacija 624 bile su uz upotrebu stroja za izvantjelesni krvotok, a 174 bez upotrebe stroja za izvantjelesni krvotok. Kod većine ispitanika ugrađene su 3 premosnice. 3,8 % operiranih pacijenata je imalo smrtni ishod, 13,2 % je zahtijevalo produženi boravak u JIL-u, a 18,5 % razvilo je perioperacijski akutni infarkt miokarda. U 87,8 % pacijenata revaskularizacija je provedena uspješno. Kod bolesnika s neuspješnom revaskularizacijom medijan dobi i EuroSCORE vrijednosti bili su viši, a medijan ejekcijske frakcije niži. Zaključak: Od ukupnog broja pacijenata više je bilo muškaraca, zastupljenija i uspješnija kirurška metoda bila je revaskularizacija miokarda uz upotrebu stroja za izvantjelesni krvotok. Kod najvećeg broja pacijenata ugrađen je manji broj premosnica nego što je bilo planirano. Bolesnici s neuspješnom revaskularizacijom bili su prosječno stariji, imali prosječno više EuroSCORE vrijednosti i prosječno niže vrijednosti ejekcijske frakcije. Uspješnost revaskularizacije razlikovala se ovisno o broju ugrađenih i planiranih premosnica.

Objectives: The aim of this study was to determine the total number of patients, their gender and age, the presence and adequacy of surgical methods, the risk factors for incomplete surgical myocardial revascularization, the difference in the success of the revascularization depending on the type of surgery, as well as the number of performed and planned bypasses. Study Design: Retrospective study. Methods and participants: Methods used in this study were review of medical history, surgical lists and hospital discharge forms for patients undergoing surgical myocardial revascularization at the Department of Cardiac Surgery in the Clinical Hospital Centre Osijek from January 1st 2010 till December 31st 2014. Results: Out of 798 participants involved, 597 were male and 201 female. Out of a total of 798 surgical revascularizations, 624 were done using an on-pump coronary artery bypass graft while 174 were done without the use of an on-pump coronary artery bypass graft. Three bypasses were performed on the largest number of patients. 3.8 % of operated patients had a lethal outcome, 13.2 % required a prolonged stay in the ICU and 18.5 % developed a perioperative acute myocardial infarction. On 87.8 % of patients revascularization was performed adequately. The age median and EuroSCORE values were higher and the ejection fraction median was lower in patients with inadequate revascularization. Conclusion: Of the total number of patients there were more males involved. Revascularization with the device for extracorporeal circulation was more effective surgical method. In the majority of patients lower number of bypasses were performed than it was initially planned. Patients with unsuccessful revascularization were older on average, had higher EuroSCORE values and lower ejection fraction values. The success of revascularization depended on the number of performed and planned bypasses.
