Abstract
Ciljevi istraživanja: Ciljevi su istraživanja utvrditi učinkovitost metode needlinga s 5-FU-om u kontroli intraokularnog tlaka te utvrditi potrebu za lokalnom medikamentoznom terapijom s ciljem kontrole intraokularnog tlaka. Ustroj studije: povijesna kohortna studija Ispitanici i metode: U istraživanje je uključeno 20 ispitanika obaju spolova kojima je učinjena revizija filtracijskog jastučića metodom needlinga s 5-FU-om. Needling s upotrebom 5-FU-a radio se u operacijskoj sali pod kontrolom mikroskopa u lokalnoj topičkoj anesteziji i uz prethodnu instilaciju antibiotika. Pacijentima se mjerio intraokularni tlak Goldmanovom aplanacijskom tonometrijom prije zahvata, jedan dan nakon zahvata te jedan mjesec i šest mjeseci nakon zahvata. Rezultati: Od 20 pacijenata revizijski zahvat na oba oka učinjen je kod njih 9. Na lijevom oku reviziju je prošlo 7 pacijenata, a na desnom 4. Prisutno je značajno smanjenje tlaka desnog oka kod kontrole nakon šest mjeseci u odnosu na mjereni tlak prije zahvata. I kod lijevog oka tlak se značajno smanjio kod kontrole nakon šest mjeseci u odnosu na prije zahvata. Smanjenje intraokularnog tlaka posljedica je provedene revizije filtracijskog jastučića needling metodom s 5-FU-om pri čemu se smanjila fibroza istog, došlo je do ponovne uspostave vodenog toka i smanjenja intraokularnog tlaka. Lokalna terapija bila je potrebna prvi poslijeoperacijski dan, mjesec dana nakon zahvata, ali i 6 mjeseci nakon zahvata, kao i masaža oka. Jednom ispitaniku 6 mjeseci nakon zahvata bila je potrebna ponovna hospitalizacija. Zaključak: Metoda needlinga s 5-FU-om pokazala se kao učinkovit način u kontroli intraokularnog tlaka, koja zahtjeva lokalnu medikamentoznu terapiju s ciljem bolje kontrole intraokularnog tlaka.
Abstract (english)
Objectives: The aims of this study are to determine the efficacy of 5-FU needling in intraocular pressure control and to determine the need for local therapeutic therapy in controlling intraocular pressure. Study design: historical cohort study Subjects and Methods: The study included 20 patients of both genders who were subjected to a 5-FU needle revision of filtration blebs. Needling with 5-FU was performed in a microscope-controlled operating room with local topical anesthesia and prior antibiotic injection. Intraocular pressure was measured by Goldman's aplanatine tonometry prior to surgery, one day after the surgery and one month and six months after the surgery. Results: Out of the total of 20 patients, 9 were operated on both eyes. Seven patients were operated on the left eye, and four on the right. There was a significant reduction in the right eye right pressure six months after the surgery. In the left eye, the pressure reduced significantly as well after six months after the treatment. The reduction in intraocular pressure is the result of a 5-FU needle revision of filtration blebs, which also lead to reduced fibrosis, re-establishing water flow and reduced intraocular pressure. Local therapy was required the first postoperative day, one month after the surgery, and 6 months after the surgery, as well as the eye massage. One patient was re-hospitalized six months after the surgery. Conclusion: The 5-FU needling method has proved to be an effective way of controlling intraocular pressure, along with localized medication therapy.
