Abstract
Cilj istraživanja: Ispitati učestalost prijeloma potkoljenice u odnosu na ukupan broj prijeloma dugih kostiju te odrediti dobnu strukturu pacijenata, tip prijeloma i metodu kirurškog liječenja. Odrediti postoje li razlike u vremenskom trajanju operacijskog zahvata između ekstramedularne i intramedularne metode osteosinteze. Ispitati koliko je prosječno trajanje hospitalizacije s obzirom na primijenjenu metodu kirurškog liječenja. Nacrt studije: Presječna studija s povijesnim podatcima. Ispitanici i metode: U istraživanju su sudjelovali bolesnici s prijelomom potkoljenice operirani od 1. siječnja 2019. do 31. prosinca 2020. godine na Zavodu za traumatologiju KBC-a Osijek. Iz elektronskih medicinskih zapisa prikupljeni su podatci o dobi, spolu, lokalizaciji prijeloma, komorbiditetu, metodi kirurškog liječenja, mehanizmu nastanka ozljede, ASA vrijednosti, razini elektrolita, hemoglobina i eritrocita prije i nakon operacije, trajanju operacijskog zahvata te dužina hospitalizacije. Rezultati: Prijelomi potkoljenice čine 24,1 % svih prijeloma dugih kostiju. Muškarci su nešto zastupljeniji (55,2 %). Najviše je distalnih prijeloma (48,3 %), u 53,1 % radilo se o lijevoj nozi dok je 84,6 % zatvorenih prijeloma. U 70,6 % slučajeva koristila se ekstramedularna osteosinteza. Nema statistički značajne razlike u trajanju operacijskog zahvata između metoda osteosinteze. Nije uočena značajna razlika u trajanju hospitalizacije. Značajno je manja razina eritrocita i hemoglobina nakon operacijskog zahvata. Zaključak: Prijelomi potkoljenice čine četvrtinu svih prijeloma dugih kostiju, najviše u distalnoj trećini. Nema razlike u vremenskom trajanju operacijskog zahvata. Duljina hospitalizacije podjednaka je kod obje operativne metode, a razina eritrocita i hemoglobina značajno je manja nakon operacijskog zahvata u odnosu na razinu prije operacije.
Abstract (english)
Objectives: The aim of this study was to determine the fracture frequency of lower leg in relation to the total fracture number of the long bones and to investigate the age and sex structure of patients, method of treatment considering the type of fracture and also duration of surgical operation and hospitalization in patients with lower leg fracture. Study design: Cross-sectional study with historical data. Patients and methods: The study included 143 patients who were treated for a lower leg fracture in Department of Traumatology in Clinical Hospital Center Osijek during two-year period (January 1, 2019 to December 31, 2020). Clinical data were collected from electronic medical records. The research data were age, gender, fracture type, injury mechanism, method of surgical treatment, ASA score, comorbidities, duration of surgery and hospitalization, levels of electrolytes, erythrocytes and hemoglobin before and after surgical treatment. Results: Fractures of lower leg account for 21,4 % of all long bone fractures. Median of age was 53 years. Men are more represented (55,2 %). The most commonly used method of surgical treatment was extramedullary osteosynthesis (70,6 %). The results showed no difference between surgical methods of extramedullary and intramedullary osteosyntesis in length of surgical operation. The average duration of hospitalization is 9 days in extramedullary and 8 days in intramedullary surgical method. Conclusion: The quarter of all long bone fractures are lower leg fractures, mostly in the distal third. With regard to gender, men are more represented. There is no difference in the duration of the surgical operation between methods. The length of hospitalization is the same in both operative methods, and the level of erythrocytes and hemoglobin is significantly lower after surgery.
